How to write school counselling session notes

Updated 2026-10-21 · By the Counseld team

Good session notes help you follow up on time, hand over cleanly, and show your principal the impact of your work. This guide gives you a simple structure that works for any school counselling session.

What should a school counselling session note include?

A useful session note records four things: the concern that brought the student in, what you observed and what the student said, the plan you agreed, and the follow-up — who does what, and by when. Add the date, the type of session, and any referral source such as a teacher.

Copy this structure into whatever you use today:

SectionWhat to writeExample
ConcernWhy the student came in, or why they were referredReferred by class teacher for low participation over the last two weeks.
ObservationsWhat you noticed and what the student shared, in neutral languageStudent was quiet at first and said they feel left out at lunch.
PlanWhat you agreed togetherPractise one conversation starter this week; join the lunchtime club on Thursday.
Follow-upNext step, who owns it, and whenCheck in next Tuesday; update the class teacher on Friday.

Stick to facts and direct quotes where they matter. Leave out opinions you would not be comfortable reading aloud to the student or their parents.

How long should a session note be?

Short enough to write in a few minutes and long enough that you, or a colleague covering for you, can pick up where you left off. A few clear sentences under each of the four headings is usually enough.

When should I write my session notes?

As soon as possible after the session, while details are fresh. Many counsellors fall behind because they wait until the end of the day. A two-minute voice note between sessions keeps the record accurate without eating into your evening.

How do I keep school counselling notes confidential?

Separate what can be shared (attendance, general progress) from confidential notes. Keep confidential notes where only you can open them, avoid putting sensitive detail in shared spreadsheets or group chats, and keep a record of who has accessed or exported them.

How Counseld writes the note for you

In Counseld you speak a voice note of up to 2 minutes, type it, or photograph a handwritten note, and it is structured into exactly these four parts: concern, observations, plan and follow-up. You can speak in English or any of 22 Indian languages, or a mix, and the note comes back in English. The audio is deleted once the note is made; it is never stored.

Notes then feed your student’s history, your calendar follow-ups and your monthly principal report, so you write each thing once.
